The front of the first page reads: "This is the Keepsake of _____ On the occasion of _____ On this the _____ day of _____ in the year _____." The back of the first page is blank. The next 29 pages are for signatures -- the front and back of each page has the header "Guests" and 10 blank lines. Also available in ivory.
